The verified legal entity, collected-data categories, affiliates, nonaffiliates, joint-marketing relationships, sharing reasons, opt-out choices, security language, and contact details are not yet approved. A final GLBA model form cannot be generated until those facts are known.

- The financial institution responsible for the notice.
- The personal information collected and the sources used.
- How information is shared for everyday business purposes, marketing, affiliates, and joint marketing.
- Which sharing a consumer may limit and how that choice is exercised.
- Approved security, contact, and joint-notice wording.
